        I seem to recall a program that could create a floppy / hd / cd
image from scratch. It could extract or build the image.
        Anyone remember this, or where to get a copy?

Are you thinking of the 'dd' command? As far as I know it is a standard
command in Unix (and Linux?) systems. A Google search indicates that Windows
versions exist.

Otherwise I use Nero to make CD / DVD images and Dave Dunfield's ImageDisk
utilities for archaic / unusual floppy disks (e.g. for my Sage IV system):
